Naphtha fire killed one Employee, severely injuring another Employee. Residual naphtha was still present in
a pipe close to a block-valve, when the adjacent flange was opened-up. The flange was opened in order to
install a blank/blind plate. Leaking naphtha found an ignition source and a huge fire caused the fatality and
severe injuries....
